Superbills for Insurance Reimbursement

Embody Health & Wellness is an out-of-network provider. This allows us the flexibility to deliver highly personalized, one-on-one care without the restrictions that often come with insurance networks. Many of our patients still receive partial or full reimbursement by submitting claims directly to their insurance providers.

We’re happy to provide a superbill — a detailed invoice of your visit — which includes all necessary codes and documentation for you to submit for potential reimbursement. While we cannot guarantee what your insurance will cover, we’ll walk you through the process and offer guidance along the way.

Medicare Coverage

If you’re a Medicare beneficiary, you may be eligible for partial or full coverage for medically necessary physical therapy. We are here to help you determine your eligibility and explain what services may or may not be covered.
